- Tracks and enters demographic and related information for care coordination

- Facilitates ongoing coordination of follow-up care to treatment and recovery support services (e.g., transportation, housing, employment, in-patient facilities, detox, mental health and/or chemical dependency assessments, primary care, food and clothing resources)
- Develops community partner relationships
- Attends trainings and meetings as required
- Complies with national Protected Health Information laws (including HIPAA and 42 CFR Part 2).
- Other projects and duties as assigned.
- Minimum of 2 years of self-attested recovery from a substance use or mental health disorder as well as 2 years of abstinence from illicit drugs and alcohol.
- 1 year of working in community health or addiction treatment.
- Ability to communicate effectively with patients and adapt to individual needs.
- Ability to communicate effectively within the organization and community agency members.
- Problem-solving and decision-making skills.
- High degree of accuracy and detail orientation in data entry.

Compensation: $22.00-$24.00 per hour
Schedule: Monday-Thursday, 40 hours a week
Ideal Option has been working on the front lines of the opioid epidemic since our first clinic opened in 2012. The Role:
As a Peer Recovery Specialist much of your time will be spent in the field assisting patients as they start recovery, representing Ideal Option at community meetings, attending events, conducting outreach, and building awareness. Additional responsibilities include:
