    We are currently looking for a Licensed Practical Nurse - Home Health to join our team!

    Key Responsibilities:

    As a member of our team, you will be responsible for administering treatments and medications, documenting patient care activities, and assisting the registered nurse in gathering health data. Collaborating with other healthcare professionals, you will play a crucial role in ensuring effective patient care delivery and achieving positive patient outcomes.

    Requirements:

    • Flexible Schedules available: Sunday-Thursday OR Tuesday-Saturday
    • Active Licensed Practical Nurse certification in the State of Florida (Required)
    • Preference for candidates with at least 2 years of recent Home Health Experience
    • Experience in other healthcare settings (e.g., SNF, Acute Care, Hospice) will also be considered
    • Experience with OASIS-D is a plus
