Job Overview:


Responsibilities:


  • Supervise and manage a team of home health aides, providing guidance, support, and direction as needed.
  • Conduct regular assessments of patient needs and progress, adjusting care plans as necessary to ensure optimal outcomes.
  • Ensure compliance with all regulatory requirements, agency policies, and procedures related to home healthcare services.
  • Provide ongoing training and education to staff members to enhance their skills and knowledge.
  • Monitor the quality of care provided by staff members through regular supervision, performance evaluations, and quality improvement initiatives.
  • Act as a liaison between patients, caregivers, and healthcare team members to facilitate communication and resolve any issues or concerns.
  • Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.
  • Participate in agency meetings, committees, and other activities to support organizational goals and objectives.

Requirements:


  • Previous supervisory or leadership experience preferred.
  • Strong communication, interpersonal, and organizational skills.
  • Strong computer literacy including proficiency in Microsoft Office Suite
  • Exceptional organizational skills with the ability to prioritize tasks effectively
  • Ability to multitask and work well under pressure in a fastpaced environment
  • Professional appearance and demeanor at all times
