    Description
    The Rescue Mission is seeking a full-time Case Manager at Whitney Place, our supportive housing facility in Binghamton, NY. The Case Manager assists residents by assessing their needs and collaborating with appropriate services to aid residents in moving toward healthy independence. This position will also assist with maintaining a structured, safe, and supportive environment that reflects the mission, vision, and values of the Rescue Mission. Responsibilities and Duties:
    • Carries caseload and maintains timely and accurate documentation.
    • Identifies strengths and barriers for each resident and completes case management assessments in a timely manner.
    • Establishes and maintains rapport with each resident. Providing supportive counseling, encouraging and motivating residents toward positive growth.
    • Assists residents with identifying attainable goals and works with them to attain those goals.
    • Monitors progress toward desired outcomes through assessment and maintaining consistent contact with each resident.
    • Reviews modifies, and follows up on resident assessments and action plans in a timely manner.
    • Collaborate with the Regional Director regarding admissions/discharges. Coordinates alternative placement as necessary.
    • Refers residents to appropriate internal and external service care providers.
    • Networks with applicable individuals, agencies, and organizations within the community.
    • Maintains client professional boundaries with clients as well as confidentiality by keeping a strict "need to know" policy and monitoring the handling of confidential information.
    • Collaborates with community services involved with each resident; financial, mental health, addiction, legal, etc.
    • Attends required meetings and events
    • Regular and predictable in-person attendance is an essential requirement of the job

    Requirements:
    • Associate degree in human services or related field or equivalent work experience
    • Bachelor's Degree in social work or related field preferred
    • Previous related work experience
    • Bi-lingual skills a plus
