    Are you a Pharmacist with at least 1 year of experience looking for a new opportunity?

    RPh on the Go is in search of a dedicated Inpatient Pharmacist for a travel position in southern Arizona. As an Inpatient Pharmacist, you will be instrumental in ensuring the safe and effective management of medications for hospital patients. Your skills and meticulous approach will directly influence patient outcomes and contribute to the overall excellence of our healthcare facility.



    Responsibilities:

    • Review and confirm accuracy, suitability, and compliance of medication orders with hospital protocols and formulary guidelines.
    • Prepare and distribute medications promptly, taking into account individual patient factors like age, weight, allergies, and medical history.
    • Collaborate with healthcare professionals to optimize medication plans and address any medication-related issues.
    • Participate in medication reconciliation processes during patient admissions, transfers, and discharges.
    • Offer drug information and education to patients, caregivers, and healthcare providers.
    • Monitor patients' responses to medications and make adjustments as needed to achieve the best results while minimizing adverse effects.
    • Utilize electronic health records (EHR) and computerized systems to record interventions and maintain precise patient documentation.

    Requirements:

    • Experience working in an inpatient hospital setting.
    • Active pharmacist license in Arizona.
