    Sign-on Bonus $3,000

    Schedule: Monday - Friday 8 am - 4:30 pm

    with weekend hours every 2 months

    The Hampton - Newport News Community Services Board is seeking a Service Coordinator to join our team in the Assertive Community Treatment (ACT) program. As a Service Coordinator, you will play a critical role in ensuring the continuity of care for our clients, who may have co-occurring intellectual developmental disabilities and substance abuse issues. You will provide essential support to our clients by addressing their physical, financial, social, and personal needs. This role includes various responsibilities such as outreach, assessments, treatment planning, counseling, and crisis intervention, with the aim of improving the well-being of our clients and helping them lead more fulfilling lives.

    Key Responsibilities:

    • Conduct outreach to identify and engage clients in need of services.
    • Perform psychological and needs assessments to develop a comprehensive understanding of clients' individual challenges and strengths.
    • Collaborate with clients to create and update personalized treatment plans.
    • Facilitate connections with service providers and coordinate access to needed resources.
    • Monitor clients' progress and adjust treatment plans as necessary.
    • Advocate for clients to ensure their needs are met within the community.
    • Provide counseling and crisis intervention when required.
    • Offer daily living support to assist clients in achieving their goals.
    • Supervise and monitor medication administration as needed.

    Requirements:

    Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling, with one (1) year of experience in case management or service coordination services. Master's degree preferred. Must be registered with the Board of Counseling to practice as a QMHP-A or register with the Board of Counseling to practice as a QMHP-A trainee within the first 30 days of employment. Valid Driver's Licenses

    Current HNNCSB are not eligible for the Sign-on Bonus


    The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
