    DaVita is looking for a Full-Time Nurse Practitioner (NP) to join us in Memphis, TN region.

    Location:
    Round and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.

    Home Office, High Speed Internet and Reliable Transportation Required.

    Position Description:

    Comprehensive office visits on the go- primary care focused visits (chair-side in dialysis clinics and in-person visits in nephrology practices).

    See and manage complex patients in a short amount of time- 30 minutes.


    Position Goal:
    To prevent hospitalizations.


    Position Details:

    • Complete Comprehensive Health Assessments (80%) of the time
    • Monday- Friday schedule with the ability to accommodate patient and nephrology partner's availability. Must be flexible.
    • Holistic management of patient conditions, including assessment, diagnosis, treatment, and consultation.

    Qualifications:

    • Minimum of two (2) years' experience as a Nurse Practitioner (NP) working in Value Based Care including documenting and coding from an MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.
    • Current unrestricted NP license to practice in the state
    • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire
    • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
    • Passion for delivering care to patients with complex, chronic illnesses
    • Home Office, High Speed Internet and Reliable Transportation Required.
    Preferred

    Qualifications:

    • Experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management
    • Spanish-speaking preferred in some markets
    • Experience in managing chronic diseases (CHF, diabetes, hypertension, diabetes)
    • Manage pop. Health (hospitalization)
