    Job DescriptionDescription:

    Receives and appropriately documents new referrals in a timely manner.

    Participates in coordinating care with nursing supervisors, patients, and the interdisciplinary team.

    Provides effective communication to patients, staff members, other healthcare professionals, and referral sources.

    Responds to the Agency and patient needs in a professional and creative manner.

    Demonstrates commitment, professional growth, and competency.

    Participates in the Agency's sponsored in-service trainings.

    Participates in the Agency's QAPI program.

    Promotes the Agency's philosophy and administrative policies to ensure quality of care.

    Requirements:

    Education: Graduate of an accredited school of vocational/practical nursing.

    Licensure: Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure

    Compact (NLC) member state. Current driver's license.

    Experience: Two year of experience as a licensed vocational/practical nurse; home health experience preferred.

    Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.

    Transportation: Reliable transportation and valid auto liability insurance.

    Environmental and Working Conditions:

    Works in a routine office environment. Noise level may be moderately high. The ability to work a flexible schedule and extended

    hours. The ability to travel locally with some exposure to inclement weather. Possible exposure to toxic materials, toner, etc.

    Physical and Mental Effort:

    Prolonged sitting and some standing required. The occasional need to lift, pull, carry, and push items weighing up to 50 pounds.

    Frequent need to stoop, kneel, and reach while accessing files. Requires working under some stressful conditions to meet deadlines

    and patient needs, and to make quick decisions. Requires hand-eye coordination and manual dexterity. Requires excellent problem

    solving skills.
