    Job Title: Home Health Intake Coordinator

    The Intake Coordinator plays a crucial role in the initial stages of patient care, serving as the first point of contact for referrals, verifying insurance coverage, and ensuring a smooth transition for patients entering our home health care services. The ideal candidate will have a strong background in healthcare administration, excellent communication skills, and a deep understanding of patient care coordination.

    Key Responsibilities:

    • Serve as the first point of contact for all incoming referrals, assessing patient payor, and eligibility for home health services.
    • Coordinate with healthcare providers, patients, and families to gather all necessary documentation for service initiation.
    • Verify insurance coverage and obtain pre-authorizations as required, ensuring compliance with all federal, state, and insurance regulations.
    • Enter and maintain accurate patient information in the electronic health record (EHR) system.
    • Coordinate and schedule initial assessments and home visits by clinical staff.
    • Provide information and answer questions from patients, families, and referral sources regarding home health services, policies, and procedures.
    • Work closely with the clinical team to ensure a seamless transition of care for the patient.
    • Participate in the development and implementation of intake policies and procedures to improve efficiency and quality of service.
    • Maintain confidentiality of all patient information in accordance with HIPAA guidelines.

    Qualifications:

    • Associate or bachelor's degree in healthcare administration, nursing, or related field preferred.
    • Minimum of 2 years' experience in a healthcare setting, home health experience preferred.
    • Strong understanding of medical terminology and insurance verification processes.
    • Excellent organizational skills and attention to detail.
    • Strong communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
