- Serve as the first point of contact for all incoming referrals, assessing patient payor, and eligibility for home health services.
- Coordinate with healthcare providers, patients, and families to gather all necessary documentation for service initiation.
- Verify insurance coverage and obtain pre-authorizations as required, ensuring compliance with all federal, state, and insurance regulations.
- Enter and maintain accurate patient information in the electronic health record (EHR) system.
- Coordinate and schedule initial assessments and home visits by clinical staff.
- Provide information and answer questions from patients, families, and referral sources regarding home health services, policies, and procedures.
- Work closely with the clinical team to ensure a seamless transition of care for the patient.
- Participate in the development and implementation of intake policies and procedures to improve efficiency and quality of service.
- Maintain confidentiality of all patient information in accordance with HIPAA guidelines.
- Associate or bachelor's degree in healthcare administration, nursing, or related field preferred.
- Minimum of 2 years' experience in a healthcare setting, home health experience preferred.
- Strong understanding of medical terminology and insurance verification processes.
- Excellent organizational skills and attention to detail.
- Strong communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
