- Patient Care:
- Provide direct patient care, including administering medications, wound care, and monitoring vital signs.
- Assist with activities of daily living (ADLs) such as bathing, dressing, and feeding.
- Implement and follow individualized care plans in accordance with physician orders and nursing standards.
- Monitor and document patient conditions, reporting any significant changes to the healthcare team promptly.
- Clinical Support:
- Collaborate with registered nurses (RNs), physicians, and other healthcare professionals to ensure cohesive and comprehensive care.
- Assist with the development and revision of patient care plans based on ongoing assessments.
- Perform routine medical procedures such as catheterization, tracheostomy care, and blood glucose monitoring.
- Patient and Family Education:
- Educate patients and their families on disease management, treatment plans, and palliative care options.
- Provide emotional support and guidance to patients and their families, helping them understand and cope with the patient's condition.
- Documentation and Compliance:
- Maintain accurate and up-to-date patient records, ensuring compliance with legal and regulatory requirements.
- Document all nursing interventions, patient responses, and significant observations in the patient's medical record.
- Quality Improvement:
- Participate in quality improvement initiatives and adhere to best practices to enhance patient outcomes.
- Stay informed about the latest advancements in nursing care and continuous care practices.
- Safety and Infection Control:
- Adhere to all safety protocols and infection control procedures to ensure a safe environment for patients and healthcare providers.
- Use personal protective equipment (PPE) appropriately and maintain a clean and safe working environment.
- Current and valid LVN license in the state of employment.
- Graduation from an accredited nursing program.
- Minimum of 1 year of nursing experience, preferably in a continuous care, hospice, home health, or long-term care setting.
- Current CPR/BLS certification.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of an interdisciplinary team.
- Strong organizational skills and attention to detail.
- Compassionate and empathetic approach to patient care.
- Ability to lift and move patients as required.
- Prolonged periods of standing, walking, and physical activity.
- Capability to perform tasks requiring manual dexterity.
- May require travel to patient homes or other care facilities.
