Overview:

Position Summary:

The Nurse Care Manager performs complex discharge planning for patients of different ages and developmental levels with serious illnesses.

Focuses on patients with complexities related to finance and/or resource utilization.

Acts as advocate for the patient/family by collaborating with the multidisciplinary team to develop and implement a case management plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner.

Participates/facilitates in the development and outcome analysis of clinical pathways as a member of the multidisciplinary CQI team.


Minimum Qualifications: 3+ years nursing experience with advanced level pediatric clinical experience and utilization management and/or discharge planning experience. Eligibility for CCS paneling if not already a paneled provider required. Patient coordination, Ambulatory/outpatient, Pediatric Nephrology, and Nephrology experience preferred.
Thorough knowledge of managed care, utilization management, discharges planning, and continuous quality improvement concepts. Computer Literacy. Strong interpersonal and negotiation skills. Patient coordination, Ambulatory/outpatient, Pediatric Nephrology and Nephrology experience preferred. Fluency in both English and Spanish desired.


Education/Licensure/Certifications:
Bachelor's Degree in Nursing required. Master's degree in Nursing, Health Care Administration, or related field preferred. Current Registered Nurse licensure in the State of

California and AHA CPR/BLS certificate required.
