- Provide support services to individuals experiencing homelessness.
- Engage with clients to assess needs, preferences, and strengths.
- Develop individualized housing case plans with clients covering various goals.
- Assist with budgeting and financial literacy.
- Establish trusting relationships with clients and ensure well-being.
- Manage medication for clients when necessary.
- Complete all necessary documentation and case notes accurately.
- Collaborate with external agencies providing services to homeless individuals.
- Participate in assessments and case meetings.
- Attend professional training and conferences.
- Assist in locating wraparound services and adequate housing resources.
- Act as a liaison with landlords and community stakeholders.
- Bachelor's degree in psychology, social work, humanities, or related field.
- Minimum 1-2 years of relevant experience working with youth in vulnerable situations.
- Proficiency in computer software programs and Microsoft Office.
- Knowledge of social service providers and public benefits programs.
- Strong communication, organizational, and case management skills.

The Case Manager is responsible for providing person-centered case management services for individuals in the Youth Transitional Housing Program at KBEC. The main objective is to help individuals overcome barriers to permanent housing using a strengths-based, Housing First approach. The Case Manager will deliver on-site case management services.
