Job Summary:


We are seeking a dedicated and compassionate Licensed Vocational Nurse (
LVN) or Licensed Practical Nurse (
LPN) to join our long-term care facility in Claremont, NH.

As an
LVN/
LPN at our facility, you will play a crucial role in providing comprehensive nursing care and ensuring the comfort and well-being of our residents.

Your primary responsibility will be to assist the Registered Nurses (RNs) in administering medications, documenting patient information, and collaborating with healthcare professionals to create individualized care plans.


Responsibilities and Duties:

  • Carry out medication administration and accurately document the administration process.
  • Monitor and record vital signs, such as blood pressure, temperature, pulse, and respiration rates.
  • Assist in the assessment and evaluation of patients' physical and mental conditions.
  • Collaborate with interdisciplinary teams to develop and implement personalized care plans.
  • Provide emotional support to residents and their families, offering empathy and understanding.
  • Administer exceptional wound care, maintain sterile techniques, and monitor healing progress.
  • Perform nursing procedures, such as tracheostomy care, catheter insertions, and tube feedings.
  • Maintain accurate and organized medical records and other required documentation.
  • Adhere to infection control policies and protocols, ensuring the safety and health of residents.
  • Communicate effectively with healthcare professionals, residents, and their families.
  • Participate in ongoing training programs and educational opportunities to enhance nursing skills.

Qualifications and Skills:

  • Current and unrestricted
    LPN or
    LVN license in the state of New Hampshire.
  • Graduation from an accredited nursing program and successful completion of the NCLEX-PN exam.
  • Proven experience in longterm care or geriatric nursing is preferred.
  • Strong knowledge of nursing principles and practices, with a focus on providing excellent patient care.
  • Proficient in using electronic medical record (EMR) systems and basic computer skills.
  • Excellent interpersonal skills and the ability to communicate effectively with residents, families, and healthcare professionals.
  • Compassionate, patient, and empathetic approach to dealing with residents' physical and emotional needs.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Outstanding organizational skills and attention to detail to maintain accurate records and documentation.
  • Flexibility to work various shifts, including weekends and holidays.
