- Bachelor's degree in social work or a comparable human service field AND four years' experience in direct care in human services OR Master's degree in social work or a comparable Human Service field AND two years of direct care experience in human services.
- Supervisory/program management experience is preferred.
- Valid Driver's License and acceptable driving record
- Prefer proficiency in MS Word and Excel
- *Provides positive leadership and promotes the trauma informed culture for all direct service staff, program clients and in the community.

- *Review caseload status of team members and assign new cases/referrals to ensure capacity.
- *Reviews and monitors client records to ensure that required information is collected, entered in prescribed formats, and reported in a timely manner prior to move-in, intake and exit.

SUMMARY DESCRIPTION OF WORK
This position provides direct supervision or program oversight in the following Support Services Programs: Street Outreach, PATH, and Homeless Diversion. These programs provide outreach and case management services to individuals in Douglas, Sarpy, and Pottawattamie County who are at risk or are experiencing street homelessness. The PATH program provides Case Management Services to individuals residing in Pottawattamie, Harrison or Mills Counties the Council Bluffs metropolitan area who are homeless, or at risk of becoming homeless, and who have a serious mental illness or a co-occurring disorder. This Homeless Diversion program serves to empower those at immediate risk of homelessness to chart their own path back to housing, and then assist them in helping them implement their plan. Homeless Diversion will identify and implement individual plans to prevent individuals and families from having to enter shelter, secure housing (if the immediate alternative to shelter isn't permanent.) Through trauma informed care we get to walk with everyone to work toward their own conflict resolution. Diversion Services include problem solving, mediation and crisis intervention services to help individuals prevent episodes of homelessness, by bolstering an individual's own ability to think for themselves, weigh and consider options and advocate for their own needs and ultimately act on their own behalf. For all three programs, services are aimed at helping individuals develop a plan to obtain permanent housing as quickly as possible.
Supervisory responsibilities and major agency wide support functions begin at this level. Decision making, problem solving, & ongoing task prioritization are critical. Training in supervision & management is required.
Compensation: between $45,600 and $57,689 per year (salary is determined by number of relevant years of experience)
Work Schedule: 40 hours per week
