    RPh on the Go is seeking a motivated and experienced Inpatient Pharmacist available for a travel assignment in North Michigan. As an Inpatient Pharmacist, you will play a critical role in ensuring safe and effective medication management for hospitalized patients. Your expertise and attention to detail will directly impact patient outcomes and contribute to the overall success of our healthcare facility.     Responsibilities:

    • Review and verify medication orders for accuracy, appropriateness, and compliance with hospital protocols and formulary guidelines.
    • Prepare and dispense medications in a timely manner, considering patient-specific factors such as age, weight, allergies, and medical history.
    • Collaborate with healthcare providers to optimize medication therapy plans and resolve any medication-related issues.
    • Participate in medication reconciliation processes during patient admissions, transfers, and discharges.
    • Provide drug information and education to patients, caregivers, and healthcare professionals.
    • Monitor patients' responses to medications and adjust therapy as necessary to achieve optimal outcomes while minimizing adverse effects.
    • Utilize computerized systems and electronic health records (EHR) to document interventions and maintain accurate patient records.


    Requirements:

    • Inpatient hospital experience
    • Active pharmacist license in the state of Michiga
