    Manager of Case Management

    St. Paul, Minnesota - Work is hybrid in office/remote

    Overview:

    Reliatus Behavioral Health is seeking a dedicated and experienced Manager of Case Management to join our team in St. Paul, Minnesota. The ideal candidate will possess strong leadership skills, a passion for helping others, and the ability to effectively manage and support our case management team. This role requires a Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.

    Key Responsibilities:

    Oversee the daily operations of the case management department, ensuring high-quality service delivery.

    Supervise and support case managers, providing guidance and professional development opportunities.

    Develop and implement case management policies and procedures to enhance service efficiency and effectiveness.

    Ensure compliance with all relevant regulations and standards in case management practices.

    Manage and resolve complex cases and issues, providing expertise and direction as needed.

    Coordinate with other departments and external agencies to facilitate comprehensive care for clients.

    Monitor and evaluate program performance, using data to inform improvements and innovations.

    Prepare and present reports on case management activities and outcomes to senior management.

    Qualifications

    Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.

    (Social Workers must be licensed in the State of MN)

    -Must have experience working in waivered programs under the Minnesota Department of health services waiver program.

    Proven experience in case management, with a minimum of 3-5 years in a supervisory or managerial role.

    Strong understanding of case management principles, practices, and regulations.

    Excellent leadership, organizational, and communication skills.

    Ability to work effectively in a fast-paced and dynamic environment.

    Proficient in using case management software and other relevant technologies.
