    JOB TITLE: Case Manager at Avivo's Intense Community-Based Services (ICBS)

    Your role:

    As a Case Manager for Avivo's ICBS, you will play a pivotal role in supporting adults dealing with mental illness and addiction. By working closely with community partners, you will develop comprehensive discharge and transition plans to help clients stabilize and prosper. This position presents a unique chance for professional growth and learning within the Human Services sector.

    MAIN RESPONSIBILITIES:

    • Conduct behavioral health intakes and interim care coordination services for clients referred by community partners.
    • Complete intakes, assess needs, develop case plans, and arrange ongoing community services.
    • Coordinate physical and behavioral health management through a holistic approach to enhance clients' overall well-being.
    • Record and communicate any changes in client status or requirements.
    • Aid clients in coordinating care with service providers.
    • Offer support and education to the significant others and family members of clients involved in their recovery.
    • Engage in case discussions with experienced team members.
