Description

JOB SCOPE:
Provides clinical services and coordination of care in an integrated community environment. Provides consultation to clinical teams for clients with dual diagnosis including Mental Health, Substance Use and Medical needs. This position requires clinical experience, and the desire to work as part of a team.


ESSENTIAL FUNCTIONS:


  • Conduct groups and individual sessions in the clinic setting and in the community.
  • Conducts home and community visits.
  • Provides ongoing clinical services to assigned caseload and provides consultation for clients who may need services or are in need of short term clinical interventions.
  • Provide services in an evidence based manner to encourage retention in services.
  • Refer to additional services as appropriate.
  • Function within an interdisciplinary team to provide high quality and evidenced based services.
  • Participates in interdisciplinary team case conferences.
  • Participates in internal and external committees.
  • Participates in internal and external CQI projects.
  • Provides supervision to students based on agency need.
  • Performs other duties, consistent with the goals and objectives of the program, as directed by Director.

Salary:
$55,000 Annually


Schedule:
Monday to Friday 9am-5pm


EDUCATION AND EXPERIENCE:


  • CASAC, Advance CASAC, Master Level CASAC (Dual Bachelor or Masters preferred).
  • Must be outcome oriented in a fast paced environment with deadlines.

KNOWLEDGE AND SKILLS:


  • CASAC (Dual Bachelor or Masters preferred)
  • Knowledge of provisions of SUD/MH services in personcentered, traumainformed and recovery oriented environment.
  • Understanding and willingness to support Medication Assisted Treatment.
  • Strong interpersonal skills with staff and colleagues throughout PCMH and ability to work as part of a team.
  • Understanding and willingness to provide services in an integrated setting.
  • Ability to verbally express ideas and concepts clearly to others.
  • Experience working with children, adults and families.
  • Proficiency with Microsoft Office.
  • Excellent written communication skills and ability to maintain charts as per policy and procedures.
  • Bilingual, ability to speak Spanish desirable.
  • Ensure progress notes are completed within 2448 hours of the delivery of the service.
