Position Purpose


The Case Management Supervisor provides guidance and supervision to a team of medical and community case managers and other social services team members.

Provides case management services to area clients both directly and indirectly.


Essential Functions

  • Provide ongoing supervision to a regional team of medical and community case managers to assure effective service delivery in accordance with all federal, state and agency standards, and which underscores access and adherence to medical care and treatment. The team supervised may include Clinic Case Managers, Community Case Managers, Linkage to Care Specialists, Outreach Case Managers, and other Social Services team members.
  • Provide supervision, guidance, coaching, and staff development for the team to implement systems of operation that assure high quality services and effective use of staff resources through a quality improvement program. Meet with staff for monthly PMIs (one on one meetings with staff) and team meetings to ensure that staff are trained, knowledgeable, and informed on agency updates, policies, goals, and progress toward goals throughout the year.
  • Ensure implementation of policies and procedures governing case management, housing, and other supportive services to promote a cohesive and multidisciplinary approach to care and treatment of patients.
  • Collect and run productivity, compliance, and access to care reporting in order to monitor adherence to State Standards and to Vivent Health's stipulated goals and outcomes. Review each case manager's monthly performance and the team's progress towards agency and departmental quality indicators, including but not limited to retention to care and HIV viral load suppression goals.
  • Provide direct case management services, to a limited caseload, according to Wisconsin AIDS/HIV Program Case Management Standards, HUD policies and Vivent Health policies
  • Maintain appropriate client files and timely documentation of services utilizing Provide Enterprise, EPIC, and other software.
  • Promote Vivent Health's integrated service delivery system through direct collaboration with the Vivent Health Medical Centers, Pharmacy, other Social Service programs, other internal departments, and key community partners, to assure efficient, effective, and highquality service delivery to clients/patients.
  • Maintain knowledge of case management, HIV/AIDS, chronic diseases, mental health conditions, AODA issues, the impact of trauma, benefits and insurance, and other relevant areas through ongoing education and learning.
  • Provide supervision to student interns and volunteers to help support case management and other social services programs.

Qualifications

  • Baccalaureate or graduate degree in social work, health care, or a human service discipline from an accredited college or university.
  • Minimum of two years' experience offering direct client contact in a social service, housing, or health service agency or clinic.
  • Minimum of one year of experience providing supervision in a social service, housing, or health service agency or clinic.
  • Demonstrated ability to supervise staff and monitor programs.
  • Ability to establish and maintain effective public and working relationships with culturally diverse populations from a wide range of life circumstances and backgrounds.
