- Serves as consultant and resource for school-based partners and outpatient pediatric clinics.
- 1)Responsible for completion of a social/behavioral services assessment on identified patients and families.
- 2)Provides advocacy, crisis intervention, counseling and problem solving in a variety of areas for patients and families to address identified needs.
- 3)Counsel individuals, groups, families, or communities regarding issues including mental health, poverty, unemployment, substance abuse, physical abuse, rehabilitation, social adjustment, child care, or medical care.
- 4)Interview clients individually, in families, or in groups, assessing their situations and capabilities to determine what services are required to meet their needs.
- 5)Evaluates quality of patient and family support network in the community.
- 6)Refer clients to community resources for services, such as job placement, debt counseling, legal aid, housing, medical treatment, or financial assistance, and provide concrete information, such as where to go and how to apply.
- 7)Participates in development of protocols and forms.
- 8)Promotes and evaluates continuous improvement in effectiveness through participation in initiatives and committees.
- 9)Establishes community-based referral partnerships.
- Serve as consultant and/or resource person in behavioral health to pediatric and family medicine residency programs.
- 1)Reflecting the Advocate Physician Partners and Advocate Medical Group's new performance metrics for pediatrics as a part of the Clinical Integration Program, educate practices on how to operationalize developmental screening and other screenings as recommended by the Bright Futures Guidelines for Health Supervision, and assist clinicians to provide support and resources for families who are "at risk" system wide.
- 2)Develops clinical/educational materials to support the clinic. Prepares materials regarding diagnosis and treatment for distribution to the professional community.
- 3)Coordinates/collaborates with practice staff on potential patients to be referred to therapy or case management.
- 4)Develops and maintains relationships with outside providers, case managers and hospitals.
- 5)Provide training and technical assistance, peer consultation and support to interdisciplinary staff.
- 6)Strengthen relationships between families and practice and increase referrals made for maternal depression, autism, domestic violence, and other family factors that affect child development to early intervention, and other community resources.
- Participation in the multidisciplinary team, brings expertise to assist in the diagnostic and treatment planning process.
- 1)Assists in program development, evaluation, reporting and implementation.
- 2)Maintains good working relationships with the medical staff and other members of the multidisciplinary team.
- 3)Participates in community and program meetings.
- Maintenance of professional standard and responsibilities for his/her own professional practice according to hospital, department, state regulations and NASW Standard and Code of Ethics.
- 1)Participate in continuing education activities to promote professional growth and development and participate in peer review.
- 2)Cultivates and maintains relationships with internal customers, community resources and the community at large.
- 3)Keeps abreast of new resources in the community.
- 4)Maintains a good working relationship with state agencies and programs.
- 5)Stays current in knowledge of legislation, regulations, and court actions impacting patient population.
- LCSW or LCPC w/ state of Illinois.
- Family Based Therapy certification(can receive within the position).Education/Experience Required:
- Master Degree in social work or counseling from an accredited program.
- Five to seven years clinical experience with at least 5 years of working with children of varying ages and 2 years' experience with behavioral health.
- Experience with working as part of a team and ability to work as part of an interdisciplinary team of professionals in health care or school based settings.
- Proficient Word, Excel and Power Point.
- Must have ability to travel to various schools and community based organizations.
