    Description


    Physical Therapist Must be a graduate of an accredited school of physical therapy with a physical therapy degree as follows Minimum of a Bachelors degree if graduated prior 1/1/2016 Doctorate of Physical Therapy required if graduated after 1/1/2016 International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelors degree in Physical Therapy.

    Doctorate of Physical Therapy degree preferred.

    ORPhysical Therapist Assistant Must have an Associates or Bachelors degree from an accredited PTA programOROccupational Therapist Must be a graduate of an accredited school of Occupational Therapy with an Occupational Therapy degree as follows Minimum of a Bachelors degree if graduated before 1/1/2002 Masters degree required if graduated after 1/1/2002 International Candidates must be a graduate of an accredited school and possess a minimum of a Bachelors degree in Occupational Therapy.

    A Masters degree is preferred.
    OROccupational Therapy Assistant Must have an Associates or Bachelors Degree from an accredited Occupational Therapy Assistant program.

    ORSpeech-Language Pathologist Must hold at a minimum a Masters Degree in Speech-Language Pathology from a school accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA).Must hold or be eligible to receive a PT/PTA/OT/OTA/SLP license in the state where services will be provided.

    State licensure must be obtained within the first 90 days of employment. SLPs must hold a Certificate of Clinical Competence from ASHA. APTA/AOTA/ASHA membership preferred.
    Must be certified in CPR or must become certified within the first 90 days of employment. Must maintain all certifications and licenses according to the expiration date.
    If assigned to drive a company vehicle a valid drivers license is required. If assigned to drive a personal vehicle proof of insurance is required in addition to a valid drivers license. OSHA certification required.

    Ergonomics certification preferred.5 years minimum in a healthcare setting with demonstrated knowledge of clinical operations within therapy that includes knowledge and/or experience with procedures equipment supplies and medications.

    Demonstrated ability to build consultative relationships with physicians co-workers leaders and external vendors. Able to provide concise complete information in both verbal and written communication. Must be willing to work at other locations as assigned.
