    Description

    Job Summary:

    Coordinates with healthcare professionals to plan and implement a comprehensive approach for managing health conditions of specific patient populations. Develops treatment plans, evaluates care, recommends care levels, and identifies cost-effective protocols.

    Essential Responsibilities:

    • Plans, evaluates, and assesses care for members.
    • Evaluates medical and psychosocial status, develops individualized treatment plans, and ensures compliance with regulations.
    • Implements diagnostic testing and interventions.
    • Creates education plans for patients and families, focusing on self-management.
    • Targets risk factors, assesses outcomes, and produces reports on patient populations.
    • Participates in achieving predetermined outcomes and develops case management policies.
    • Coordinates care with various providers, manages levels and quality of care, and provides continuity of care.
    • Consults with healthcare providers, agencies, and coordinates follow-up care.
    • Encourages adherence to care plans and makes referrals to community services.
    • Identifies cost-saving opportunities and collects utilization data.
    • Interprets regulations, benefits, and acts as a liaison for outside agencies.
    • Coordinates treatment information transmission and monitors quality of care.

    Basic Qualifications:

    Experience: N/A.

    Education: Graduate of an accredited institution by the Council on Social Work Education with a Master's degree in Social Work.

    License, Certification, Registration: N/A

    Additional Requirements:

    • Proficiency in case management and regulations.
    • Ability to work in a collaborative environment.
    • Bilingual proficiency in English and Spanish Level II required.

    Preferred Qualifications:

    • Minimum two years of case management experience preferred.
    • Current and valid LCSW highly preferred.

    Notes:

    • Position for Long-term Care with possible transition to Bridge Clinic.
    • Successful bilingual test within the last 12 months required.

    Primary Location: California, Bellflower, Bellflower Medical Offices

    Hours Per Week: 40
    Shift: Day
    Workdays: Mon, Tue, Wed, Thu, Fri
    Working Hours: 08:30 AM - 05:00 PM
