    Job Description


    The Christ Hospital Post-Graduate Year 2 Cardiology Pharmacy Residency Program is an organized, directed, postgraduate program intended to develop a highly trained pharmacist with advanced knowledge and skills in the application of drugs and drug therapy to cardiology pharmacy practice.


    Responsibilities

    Participates in Patient Care Activities.


    • Assess the appropriateness of medications on the basis of health conditions, indication, and therapeutic goals; evaluate the effectiveness, safety, and affordability of medication therapy in respect to patient specific factors such as age, weight, laboratory data and other procedures; identify medication-related problems. Correlate analysis with direct patient observation and findings from the patient's physical exam and communicate analysis with other clinicians and practitioners responsible for the patient's care. 20%
    • The resident, when functioning in a patient care area, takes responsibility for the continuity of care of a specific patient or team over the entire course of a hospitalization or phase of a disease. 10%
    • Provide written and/or verbal consultation related to (but not limited to) intravenous therapy, clinical pharmacokinetics, selection and/or continuance of drug therapy, and determination of therapeutic end points. 15%
    • Provide documentation of all pharmacy related interventions. 5%
    • Provide education and counseling to patients regarding their medication(s) when appropriate. 10%
    • Participate in cardio-pulmonary resuscitation efforts (when applicable). 5%
    • Participate in drug distribution activities during periods of staffing shortage or as scheduled to maintain continuity of departmental services. 10%
    • Provide expert consultation with other health care professionals. Therefore, the resident must be thoroughly familiar with the hospital's formulary and all department policies and procedures in order to be a resource person for others. 10%
    • Demonstrate excellent assessment and communication skills on a regular basis. 10%
    • Position requires ability to meet age-specific pharmaceutical needs for all ages (newborn, infant, adolescent, adult, geriatric) with primary focus on adolescent, adult and geriatric population. 5%

    Participate in Indirect Patient Care Activities


    • Participate in Multidisciplinary teams and committees to develop clinical pathways and treatment guidelines in order to promote continuous quality improvement and clinical effectiveness. 10%
    • Participate in drug usage evaluations and continuous quality improvement activities, including but not limited to adverse drug reaction reporting. 25%
    • Resolve problems relating to drug distribution as identified and/or referred for nurses or decentralized supportive personnel. 40%
    • Participate in the evaluation of drugs and preparation of monographs for the Pharmacy and Therapeutics Committee. 25%

    Participates in Educational Activities.


    • Participate in the education of pharmacy personnel. 40%
    • Participate in the education of non-pharmacy personnel including physicians, nurses and respiratory therapists. 50%
    • Participate in the didactic education of undergraduate students and PharmD. Students.
    The practitioner must serve as a role model for all health professionals. 10%


    Participates in Research Activities.


    • Develop and complete a residency project. 80%
    • Participate in ongoing pharmacy research projects. 20%

    Participates in Inpatient Pharmacy Activities.


    • Reviews, prioritizes and problem solves physician orders using patient profiles to ensure medications prescribed are the appropriate dose; drug-drug, drug-disease, and drug-food interactions, incompatibilities, and patient allergies are avoided; and patient outcomes are achieved using the most effective agent. Monitors medication administration profiles to verify proper route, dosing and timing of medications transcribed. 25%
    • Accurately dispenses properly labeled medications to both inpatients and outpatients. 5%
    • Prioritizes and monitors daily operational duties in assigned areas using support personnel in the most effective manner, including coordination of break and lunch (dinner) schedules, to ensure patient care services are maintained and provides assistance to technical staff. 5%
    • Responds to drug information requests concerning the availability, content, form, side effects, dosing, drug interactions, and possible incompatibilities of new as well as currently used drugs. 10%
    • Performs consults when necessary and documents all recommendations and interventions appropriately. 40%
    • Properly maintain all records as required by state, federal and Department of Pharmacy regulations for the dispensing and disposition of narcotics, other scheduled drugs, dangerous drugs and Investigational drugs. 5%
    • Accurately weighs, measures and mixes ingredients; compounds and prepares parenteral and oral medications, emulsions, liquids, powders and ointments using proper equipment and technique. 5%
    • Participates in the pharmacologic therapy and provides the necessary medication and equipment during a code situation. 5%

    Qualifications

    EDUCATION:
    B.S. in Pharmacy or Doctor of Pharmacy (Pharm.
    D.) from ACPE accredited college of pharmacy.

    Completion of a PGY1 ASHP-accredited or pending accreditation program
