Description
Duties include but are not limited to:
Completing the appropriate nursing process, following nursing standards of care for every resident in the Substance Abuse Residential Rehabilitation Treatment Program (SARRTP) and Psychosocial Residential Rehabilitation Treatment Program (PRRTP).
Functioning as Case Manager, creating individualized plans for SARRTP and PRRTP residents, coordinating nursing care, and identifying barriers to education.
Identifying and prioritizing residents' problems and needs. Continuing ongoing assessments, required re-assessments, and addressing any changes. Documenting when resident conditions change, their response to interventions and treatments.Setting measurable goals, objectives, and outcomes with the resident.
Presenting residents for review at expanded team meetings.
Providing resident/family education based on the identified needs and problems assessed.
Completing transfers and/or discharge summaries.
Monitoring the therapeutic milieu, maintaining a safe respectful environment of interaction between residents of various programs.
Communicating with Primary Mental Health Provider, Medical staff, Addiction Therapists, Psychosocial Education (PSE) staff, family or significant other, and VA Police as needed to facilitate care and safety.
Communicating to charge nurse or appropriate other personnel potential situations in which a resident may be a danger to self or others.
Ensures direct care staff provide safe and appropriate care. Follows local policies.Serving as the resident's advocate, problem solves issues at the lowest level, collaborating with charge nurse and/or care coordinator.
Transcribing and verifying orders accurately and timely. Administering medications and treatments according to VHA directives and local policies.Conducting group trainings and one-to-one activities as assigned.
Reviewing assessments completed by others, and obtains missing information from providers, patients and medical records.
Maintaining forms, surveys, instruments, and materials for proper coordination of the different treatment modules available through the Behavioral Health Program.
Providing psychoeducation and direct advice regarding symptoms of depression, Post Traumatic Stress Disorder (PTSD) and alcohol misuse of residents.
This includes the development of a collaborative treatment plan for residents and the ability to communicate that plan to the resident's provider.
This position is eligible forthe Education Debt Reduction Program (EDRP),
a student loan payment reimbursement program.
You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
