    Job DescriptionA home health agency located in Lake County, IN is currently seeking a Registered Nurse to join their team full timeCheck out the details below:

    Title: Registered Nurse

    Job Type: Permanent / Direct Hire

    Setting: Home Health

    Location:Lake County, IN

    Schedule: Full Time, self-scheduling M-F, no weekends.

    Pay: Competitive pay based on experience plus a full benefits package


    The Registered Nurse is responsible for performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.

    Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy unless other arrangements are made.

    Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings. Provides skilled interventions with a focus for achieving realistic client outcomes within a specified time period. Reports changes and information necessary to modify and update the care plan to reflect progress towards goals.

    REQUIREMENTS:


    • Current Indiana RN license
    • Minimum of 1 year of experience in an acute care setting
    • Current CPR certification required
    • Valid Drivers License and proof of insurance required
