    Description

    Registered Nurse (RN) Case Manager

    Opportunities in: Fort Collins, CO

    Compensation: $30-$150 per visit (dependent on visit type), PLUS mileage and travel compensation with on-call requirement

    As a Home Health Registered Nurse, here's a broad overview of your responsibilities:

    • Deliver in-home care to patients dealing with various health conditions post-hospital discharge
    • Collaborate with physicians and supervise LPNs, CNAs, and aides in delivering care
    • Conduct patient assessments, document progress, and communicate changes to physicians
    • Educate patients on their care plans, medications, therapies, diet, and exercise
    • Provide guidance to family members on the patient's care plan, medications, and home safety
    • Ensure care goals are achieved and coordinate discharge planning

    Essential requirements for Home Health Registered Nurses:

    • Active RN License in Colorado
    • Graduate from an accredited nursing program
    • Minimum of 1200 hours of RN experience
    • Current CPR/BLS certification for healthcare (proof of course completion required)
    • Preferably experience in home health care
    • Strong organizational abilities
    • Capability to manage patients with chronic illnesses
    • Successful completion of background check and drug screening (includes THC testing)
    • No mandatory Covid vaccination
