- Coordinate with other project staff and partners to provide housing placement services and supportive services to facilitate client's successful transition to permanent housing and coordinate services to foster housing stability during and beyond interim housing.
- Assist clients with addressing their immediate needs (e.g., access to emergency shelter, food, water, hygiene products, clothing, etc.)
- Connect with DPSS eligibility worker to enroll client in Medi-Cal, GR and/or any other DPSS administered services (e.g., In-Home Supportive Services (IHSS),Cal Fresh).
- Connect clients to the homeless system of care (Coordinated Entry System), entry into HMIS, entry into CRM, and referrals to housing resources.
- Monitor and follow-up with clients both County and non-County providers and partners to confirm timely completion of linkages, services, and/or housing.
- Help arrange transportation for clients to official appointments utilizing public transportation, hired/ride share services when appropriate, or directly providing transport to official appointments for those clients needing accompaniment.
- Document client's changes in health and/or mental health condition. Engage with client's health and/or mental health provider if there is a decline in health.
- Monitor and follow-up with clients, once client has achieved their care plan recommendations within outlined timeframe to advance client to the next step in ICMS BAS.
- Provide clients with direct financial assistance to obtain goods or services needed to obtain benefits and/or housing (e.g., DMV fee for new ID)
- Assist clients obtain additional services needed including but not limited to mainstream benefits, housing linkages, substance use and mental health services, employment and vocational services, education, legal services, credit counseling, money management, tenancy education, landlord relations, life skills and family reunification.
- Work to locate housing and secure housing opportunities.
- Attend weekly case conferencing meetings.
- Collaborate and coordinate with DMH, DPSS, LAUSD, Work Source, and SUD providers.
- S/he will assist clients with identifying the most appropriate housing intervention.
- Coordinate with other FMWC staff and collaborative partners to provide ancillary services to achieve housing stability; follow-up with clients weekly, at minimum; maintain interagency consultation, coordination, and referrals as it relates to housing.
- Maintain up-to-date and accurate documentation in client files for service coordination/case management, housing placement, rental assistance, and move-in assistance.
- Staff must update information, track services (i.e., Housing Stability Plan creation/updates) and referrals, and complete case notes on services provided to the participant within 24 hours following the provision of the services.
- Case Notes to be entered within 24 hours in DAP or SOAP form into the CRM and HMIS system. Additionally, case notes will be detailed and grammatically correct.
