    Description
    Receives referrals and requests for services and assigns them to appropriate staff members. Evaluates appropriateness of clients for home care.

    • Evaluate and provide oversight of referrals to ensure patient referrals are appropriately accepted, scheduled and admitted to care.
    • Supervises home care services and evaluates patient care provided by staff under his/her direction to ensure standards of practice are maintained including documented supervisory home visits with staff on an annual basis.
    • Manages the productivity, RN utilization, average caseloads, and acuity of caseloads of clinical staff.
    • Evaluates directly the clinical competence of nursing personnel. Meets with staff regularly.
    • Assures the development, implementation, and updates of the patients individual plan of care.
    • Fosters coordination and collaboration with Guilford office across all disciplines.
    • Assures coordination of care and that patient needs are continually assessed/met.
    • Assure that clinical documentation meets COP and State regulatory requirements for reimbursement.
    • Supervises clinical secretary and scheduler staff support activities involving documentation, scheduling, and other areas.
    • Facilitates case management and coordination of services; interprets and clarifies policies.
    • Promotes learning and professional growth of staff by providing orientation and ongoing feedback, ensuring availability of educational programs, and performing annual competency evaluations.
    • Participates in development of agency objectives, standards of care and clinical policies and procedures.
    • Responsible for maintaining appropriate staffing levels and for recruitment and staff selection.
    • Actively assists in program development.
    • Assesses and adjusts the caseload of each nurse under his/her direction to provide a reasonable distribution of work and opportunity for new experience to foster professional development.
    • Prepares and manages departmental budget in accordance with financial guidelines.
    • Collaborates with other departments to promote smooth processes.
    • Other duties as required.
    • Strong leadership, critical thinking and communication skills
    • Creative and highly organized and motivated team player
    • Registered nurse with active valid license in State of Connecticut
    • Masters degree from a program approved by the National League for Nursing or the American Public Health Association with a minimum of one (1) year full-time clinical experience in a home health agency or related community health program which included care of the sick at home.
      OR
    • A baccalaureate degree in nursing and minimum three (3) years of full-time clinical experience in nursing, at least one (1) year of case management experience.
