- Deliver in-home care to patients dealing with various illnesses, injuries, and chronic conditions—often post-discharge from medical facilities
- Collaborate with a team of physicians and supervise LPNs, CNAs, and aides involved in patient care
- Conduct patient assessments, document progress, and communicate any changes to the attending physician
- Educate patients on their care plan, prescribed medications, therapies, diets, and exercises
- Provide guidance to family members on the patient's care plan, medications, and home safety
- Ensure treatment goals are achieved and facilitate patient discharge from services
- Active Registered Nurse (RN) License in Colorado
- Graduate from an accredited nursing school or program
- Minimum of 1200 hours of RN experience
- Current Healthcare CPR/BLS Certification (with a demonstration course prerequisite)
- Preferably, prior experience in home health care
- Strong organizational abilities
- Capable of managing patients with chronic conditions
- Successful completion of a background check and drug screening (THC testing mandatory)
- No compulsory Covid vaccination
