Status: Full time or PRN

As a Home Health Registered Nurse, here's an overview of your responsibilities:

Deliver in-home care to patients coping with ailments, injuries, and chronic conditions—typically following discharge from a medical facility, rehabilitation center, or nursing home
Collaborate with a team of physicians and supervise LPNs, CNAs, and aides involved in patient care
Conduct patient assessments, document progress, and communicate any changes to the overseeing physician
Educate patients on their care plan, prescribed medications, therapies, dietary requirements, and physical activities
Guide family members on the patient's care plan, medications, and household safety measures
Ensure treatment goals are achieved and facilitate the patient's discharge from services

Essential qualifications for Home Health Registered Nurses:

Active Registered Nurse (RN) License in the state of Colorado
Graduate from an accredited nursing school
Minimum of 1200 hours of experience working as an RN
Certified in healthcare CPR/BLS (requires completion of a demonstration course)
Prior experience in home healthcare is desirable
Successful completion of a background check and drug screening (mandatory THC testing)
No COVID vaccination mandated
