As a Home Health Registered Nurse, here's an overarching view of your responsibilities:

Deliver home-based care to patients with illnesses, injuries, and chronic conditions—often post discharge from a medical facility or nursing home
Collaborate with a team of physicians and supervise LPNs, CNAs, and aides delivering patient care
Assess patients, conduct evaluations, document progress, and communicate changes to their physician
Educate patients on their care plan, prescribed medications, therapy, diet, and exercise
Provide guidance to family members on the patient's care plan, medications, and home safety
Ensure objectives are achieved and arrange discharge from services

Essential requirements for Home Health Registered Nurses:

Active Registered Nurse (RN) License in Colorado
Graduated from an accredited nursing school
Minimum of 1200 hours of experience as an RN
Healthcare CPR/BLS Certified (mandatory demonstration course)
Preference for home health care experience
Clearance of a background check and drug test (THC testing mandatory)
No Covid Vaccination Required
