- Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.
- Assist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant.
- Develop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.

- Complete all assessments and documentation as required by RHD, funder, licensing/ accreditation entities.

- Document treatment contacts using collaborative documentation.
- Provide 24 hour on call services and crisis intervention on rotating basis.
- Provide 75% of treatment contacts in the community.
- Provide culturally sensitive services that are individualized to client needs and reflect the integration of their race, color, religious creed, disability, handicap, ancestry, sexual orientation, gender expression, gender, national origin, age and sex.
- Maintain confidentiality and adhere to HIPAA
- Bachelor's degree in human services or related field.
- Intermediate computer skills.
- One year Case Management experience preferred.
- One year vocational/employment experience preferred.
- Two years' general work experience providing support to individuals in behavioral health, Addiction Recovery or IDD services required.
- Valid driver's license.
- Must pass federal and state background checks.

RHD is actively seeking a full-time Case Manager to join our Delaware ACT teamAssertive Community Based Treatment (ACT) is an interdisciplinary team that ensures a recovery-oriented system of care, with full range of treatment functions, 24 hours a day, 7 days a week. The program employs master's and bachelor level counselors, a registered nurse, a substance abuse specialist, a vocational specialist, a psychiatrist/APN, a peer specialist, a program assistant and a program team leader. Team members work together with clients to promote symptom stability and appropriate use of medication to restore personal lives, community living, and social skills; promote and maintain physical health; establish access to entitlements; housing, work, and social opportunities. The goal is to promote and maintain the highest possible level of functioning in the community. Position SummaryThe case manager will determine resources needed to meet the individualized needs of the participant, specifically around promoting working as a form of treatment using the Supported Employment Model. The case manager will take all necessary actions in assuring the implementation of the service treatment plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed.
