    Responsibilities


    General Purpose:


    Working within the department of OB/GYN, the behavioral health care manager provides integrated behavioral health care to pregnant and postpartum patients with identified depression and anxiety.

    The care manager works in consort with the OB provider and consulting psychiatrist to assess and treat non-complex behavioral health conditions using evidenced based medication and brief-psychotherapy modalities.

    The successful candidate will be skilled in collaborating with a multidisciplinary team, and experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.

    The candidate should hold a master's degree in a mental health-related field (LCSW, LMSW, LMHC, LMFT) or be a registered nurse with relevant behavioral health experience.

    Ideally, candidate will possess experience with assessment and treatment planning for common mental health disorders, and working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., behavioral activation or anxiety management).

    Additionally, the candidate will demonstrate ability to effectively engage patients in a therapeutic relationship by phone, video and in person.


    Specific Responsibilities:
    Under general direction and with considerable latitude for independent judgement within established procedures:


    • You will work alongside OB/Gyn providers to support patients with their mental health.
    • In this role, you will support and coordinate the mental health care of patients, collaborating with the patient's medical provider and any additional health providers when appropriate.
    • Screen and assess patients for common mental and health disorders.
    • Facilitate patient engagement and follow-up care like education about common mental health disorders and the available treatment options.
    • Systematically track treatment response and monitor patients (via video, in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
    • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
    • Provide or facilitate in-clinic or telehealth evidence-based psychosocial treatments as clinically indicated.
    • Participate in regularly scheduled caseload consultation with the psychiatric consultant and communicate resulting treatment recommendations to the patient's medical provider.
    • Consultations will focus on patients new to the caseload and those who are not improving as expected under the current treatment plan. Case reviews may be conducted by telephone, video, in person or via Electronic Health Record.
    • Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, mental health specialty care).
    • Provide brief, in-person individual interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, parenting support, or other treatments as appropriate.

    Qualifications:

    • Master's degree from a Counsel for Accreditation of Counseling and Related Education Programs (CACREP) accredited program with major course work in mental health counseling.
    • Requires NYS licensure as a mental health practitioner (e.g., Mental Health Counselor, Creative Arts Therapist) or limited permit eligible.
    • If licensure has not been obtained upon employment, the individual has 36 months to obtain the NYS license.

    Pay Range

    Pay Range:
    $57,595 - $80,600 Annually

    The referenced pay range represents the minimum and maximum compensation for this job.

    Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
