    NATURE OF WORK Under close supervision performs work of routine difficulty in providing health care education in clinics and homes; performs related work as required or assigned.

    This position has department specific responsibility.

    Example of Work(Illustrative Only) Assists with identifying, recruiting and training high-risk individuals to attend health care classes; maintains class records, rosters, registration and follow-up forms; teaches culturally appropriate, effective health promotion and disease prevention classes and workshops for target population; maintain a current list of community resources for patients and their families to maintain and improve their health status; participate in community health fairs and events, networks with community organizations, schools, and outside agencies to provide information on available public health resources in the community; develops, prepares and conducts community outreach and recruitment to enroll individuals in health department programs; acts as advocate and makes appropriate health /community referrals; attend regular staff meetings, trainings, and other meetings as requested; conduct complete, accurate, and timely data entry about activities and outreach; maintains HIPP compliance at all times; regular and reliable attendance is required; performs other duties as assigned.

    Experience and EducationHigh School Diploma or GED equivalent; six (6) months of health education experience; OR an equivalent combination of education and experience; must possess or have the ability to obtain a fingerprint clearance card within 3 months of hire; must possess and maintain a valid Arizona driver's license; requires successful completion of a background check.


    SPECIAL REQUIREMENTS:
    Successful completion of required training program and must possess and maintain a valid Arizona driver's license.
    Hourly rate for this position has been revised effective 3/4/24.

    Knowledge, Skills and AbilitiesKnowledge of County policies and procedures; basic health care practices; community health services; community resources for appropriate referrals.


    Skill in:
    Proper health care techniques; recognizing and reporting patient health status; establishing and maintaining effective working relationships with employees, other agencies and the public; following written and verbal instructions; communicating effectively verbally and in writing; organization and establishing priorities; use of personal computer and related software applications, including email


    Ability to:

    Read and interpret documents such as policies and procedures; write reports and correspondence; use modern office equipment and computers; lift at least 5 to 25 lbs.

    Ability to work with diverse populations; communicate effectively verbally and written; perform the essential functions of the job specifications with or without a reasonable accommodation Work EnvironmentThe usual and customary methods of performing the job's functions require the following physical demands:

    occasional lifting, carrying, pushing, and/or pulling; some stooping, kneeling, crouching, and/or crawling; and significant fine finger dexterity; generally the job requires 60% sitting, 20% walking and 20% standing; working at a computer monitor for extended periods required.

    The job is performed under minimal temperature variations and in a generally hazard free environment.

    The employee is able to select from three (3) medical plan options with six (6) tiers that will best suit the employee's needs, dependents or families.


    Plan Option 1:
    Preferred Provider Option A (PPO) or PPO A 900 deductible with $10.00 Wellness Incentive. Employees who participate in the "Wellness Incentive" will receive a $20 monthly premium reduction.

    Plan Option 2:
    Preferred Provider Option B (PPO) or PPO B 1,200 deductible with $10.00 Wellness Incentive. Employees who participate in the "Wellness Incentive" will receive a $20 monthly premium reduction.

    Plan Option 3:
    High Deductible High Plan
    • Health Savings Account (HDHP-HSA) or HDHP-HSA with Wellness Incentive.
    HDHP-HSA Wellness Incentive for Employee Only option can receives upto $240.00 annual contribution to a Limited Purpose FSA card.
    Southwest Service Administrator (SSA)

    Mexico PPO Health Plan:
    SSA is available to employees who are enrolled in YCEBT Medical PPO Plan.

    The SSA Plan pays 100% of hospital, medical, laboratory and prescription drug benefits, to a maximum of $5,000 per person per calendar year ($10,000 for two or more family members).This position may be retirement eligible, if you have any questions contact Recruitment division at or visit for more information.
