_As a team, we uniquely help individuals experiencing homelessness in Maricopa County.

Our work is based in downtown Phoenix, but reaches across the Maricopa County, working to end homelessness for one of the fastest unhoused and unsheltered populations in the United States.


Summary


The Multi-Disciplinary Street Outreach Team works with Human Services Campus partner agencies to engage and connect those experiencing homelessness across Maricopa County to services and solutions to end their homelessness.

The target population is unsheltered, and on the street. Outreach will be mainly outside or in emergency shelter settings in a set geographical area.

Outreach Team members will connect the client to appropriate services such as healthcare services, shelter, financial assistance, and other programs that will assist the client with resolving their homelessness and staying housed.


What You Will Do To End Homelessness

  • Work with partner agencies and management to determine key geographic areas to outreach daily.
  • Work with management to identify key clients needing to connect to services
  • Provide services in a manner congruent with harm reduction theory, progressive engagement, lowbarrier eligibility, trauma informed care, and national best practices.
  • Conduct vulnerability assessments (VI-SPDAT and SPDAT) as needed.
  • Collect client demographic information and obtain appropriate releases of information.
  • Experience working with homeless or other vulnerable populations.
  • Must be able to effectively communicate with clients from diverse backgrounds, including those who are experiencing mental health, substance use, traumatic, and medical barriers.
  • Must be able to work in a fastpaced environment; work as part of a team; use sound judgement; demonstrate personal responsibility and integrity; show initiative and an ability
to work interconnectedly.

  • Sensitivity to cultural diversity, lifestyle issues is essential.
  • This position is 75% outdoors. Applicant must be willing to work in high heat and inclement weather situations on the streets in an assigned geographical grid.
  • Effective written and verbal communication skills (Spanish a plus).
  • Valid Arizona State Driver's license as this position will have a significant amount of travel involved.
  • Visual acuity sufficient to maintain system of records and reports containing computer generated and handwritten documents.
  • Ability to handle sensitive material, maintaining the highest level of confidentiality and displaying sound judgement.
  • Mobility sufficient to conduct regular duties within a normal office environment.
  • Ability to lift up to 20 pounds in order to safely manage office supplies and equipment.
  • Screen for eligibility for SOAR and employment services, providing referrals when appropriate.
  • Enter information into the Homeless Management Information System.
  • Works closely with HSC staff and partners providing entry point, inreach, outreach, engagement and housing services as needed.
  • Other duties as assigned.

You're An Ideal Fit If You Meet the Following

  • Experience working with homeless or other vulnerable populations.
  • Must be able to effectively communicate with clients from diverse backgrounds, including those who are experiencing mental health, substance use, traumatic, and medical barriers.
  • Minimum of 1 years' experience in intensive case management and outreach, preferably with individuals experiencing homelessness.
  • This position is 75% outdoors; willing to work in high heat and inclement weather situations on the streets in an assigned geographical grid.
