ESSENTIAL FUNCTIONS

  • Coordinate case management and family reunification services for children.
  • Initiate and maintain direct contact with each child and the child's family while the child is in funder/oversight care in order to efficiently advance the child's family reunification and release the child from the facility.
  • Facilitate the timely release or discharge of children to identified family members or authorized caregiver/custodian and document the provision of services in each child's case file.
  • Conduct and document funder/oversight assessments in accordance with funder/oversight policies and procedures.
  • Report significant incidents in accordance with funder/oversight policies and procedures.
  • Maintain the client case file and funder/oversight database in accordance with funder/oversight policies and procedures.
  • May travel (including out of state) with unaccompanied minor to new placement.
  • Models Center's expectation for sensitivity and competency relating to diversity and cultural awareness.
EDUCATION / EXPERIENCE

  • Bachelor's degree in social work, psychology, sociology, or other relevant behavioral science.
  • Minimum one year experience in case management.
  • Clean criminal background check.
  • English / Spanish bilingual capabilities required.
  • Must possess valid Level One Fingerprint Clearance Card or able to obtain one prior to employment
  • Must be at least 21 years of age
  • Must possess a valid driver's license with a 5year clean driving record
