As part of the "TK-TelefonCoach" project, we support chronically ill TK-insured persons in adapting their lifestyle to their illness, as a health-conscious lifestyle and compliance have a significant impact on the disease.

For our Hamburg branch, we are looking for you as a health coach (m/f/d), initially limited to two years (with subsequent permanent employment).

Your job will involve providing telephone coaching to support and accompany TK-insured persons with chronic diseases (CHD, atherosclerosis, type 2 diabetes) in adapting their lifestyle to their illness.

Together with your coachees, you will develop individual goals, support their implementation, and develop alternative actions for a health-conscious lifestyle.

You have completed your studies in psychology, (social) pedagogy, or social work successfully or have a comparable qualification with relevant professional experience.

You can demonstrate practical experience in the field of professional communication, particularly in counseling or therapy for adults. You have a solid medical knowledge of the above-mentioned medical conditions. In addition to your strong communication and social skills, you ideally have knowledge in "motivational interviewing". You are proficient in using a PC and common MS Office applications. You are available for your coachees until 8 pm at least two days a week.
