Job description
Must have experience working in a medical hospital setting with an interdisciplinary team & discharge planning.


Responsibilities:


  • Conduct assessments to understand patients' social and emotional needs.
  • Provide counseling and support to patients and families.
  • Connect them with community resources and services.
  • Advocate for patients' needs within healthcare systems.
  • Coordinate care plans with healthcare teams.
  • Offer crisis intervention and endoflife support.
  • Educate patients on their conditions and options.
  • Maintain accurate records and adhere to ethical standards.
  • Engage in professional development and research.

Requirements:


  • Minimum 2 years of social worker experience.
  • Minimum 1 year of Hospital experience.
  • LSW or MSW (IL) license in hand

Job Types:
Contract, Temporary


Pay:
$2, $2,443.00 per week

  • 5x Day shift
  • Morning shift
