Duties include:

communicate with referral sources (primarily major hospitals) and patients/families, intake and scheduling patients, coordinate with clinical field staff, PCP clinics, QA staff, and biller.

Oversee intake and scheduling support staff. Requires minimum 7 years experience as RN, 2+ years in hospital discharge case management. ER/ICU experience preferred. Reports to agency administrator and supervisor. Full time in office with infrequent supervisory visits to patient homes. Intermittent weekend work with compensatory time off. Strong communication skills and patience needed. Pay commensurate with experience and is salaried. Other benefits negotiable. Secure parking on site. Friendly office environment. $95, $110,000.00 per year
