We are looking for a qualified caregiver (m/f/d) to join our team in Avon Lake, OH for our care and family counseling services.

Your responsibilities will include:


  • Preparation, execution, and followup of counseling sessions, particularly in accordance with §45 SGB XI, as well as support with classification and upgrading to a care level.
  • Facilitating the admission process for new clients.
  • Providing counseling services over the phone, via email, and in the clients' homes.
  • Assisting clients and their families in finding solutions.
  • Coordinating and arranging for nursing and therapeutic services within our facilities.
  • Referring clients to external medical and social support services.
  • Maintaining a respectful and supportive relationship with clients, their families, and service providers.
  • Collaborating with partners and other healthcare facilities.
  • Building trust with our clients (patients) and their caregivers through your compassionate approach.

We are looking for someone who:

  • Has completed training as a geriatric nurse, health and nursing care professional, or a degree in nursing (e.g. Bachelor of Nursing Management/Nursing).
  • Qualifications for counseling according to §45 SGB XI are desirable, but
    not necessary. We are willing to provide the necessary training during your onboarding process.
  • Has knowledge of SGB V and SGB XI or is willing to acquire this knowledge.
