Overview:

This is an on-site role.

The Quality Assurance Nurse:

provides clinical guidance, evaluates clinical aspects of cases, and collaborates with all Department to ensure high quality practices and ongoing performance improvement.

Conducts death chart audits (medical record reviews) in assigned hospitals in LiveOnNY's service area. The purpose of the reviews is to identify organ donor potential and barriers to the donation process.

Supports and fosters the mission of the organization and its commitment to organ and tissue donation.


Conducts real time review of active cases to specifically identify if the case was reported to LiveOnNY in a timely manner.

Reviews and trouble shoots any obstacles, communicating and reporting barriers and issues to the appropriate Department(s) at LiveOnNY.

Improving relationships with Hospital Partners to improve reporting accuracy and timeliness.


Responsibilities:


Quality Assurance Nurse Responsibilities include:

Participate in all aspects of clinical quality monitoring and performance improvement in areas including but not limited to:

  • Death Record Review and data abstraction
  • Synchronous chart review
  • Lab findings
  • Potential and actual disease transmission
  • Operational errors related to clinical activities
  • Management of deceased patients
  • Management of procured organs
  • Clinical activities related to research
  • Conducts death chart audits to identify missed organ donor potential in assigned hospitals.
  • Conducts Synchronous chart reviews to identify delayed reporting
  • Identifies cases with ICD codes to be measured by CMS and potential coding revisions as appropriate
  • Communicates all findings to the appropriate Department to ensure feedback to Hospital or agency
  • Accurately completes medical record review forms and submits all appropriate
  • Establishes procedures and tracking mechanisms to perform reviews on a monthly, quarterly, and yearly
  • Establishes and maintains good working relationships with Medical Record Departments and other customers in service area hospitals.
  • Thinks and behaves in ways that lets all customers and coworkers know they are valued, consistently demonstrating respect and patience in all interactions.
  • Maintains accurate data and status reports on complete and pending
  • Reports status of reviews to department leadership.
  • Conducts orientation and training of new hires, as appropriate, in the Medical Record Review
  • Consistently demonstrates initiative to continuously improve reporting accuracy and timeliness.
  • Tracks and trends pertinent metrics and communicate data in a clear and meaningful way.
  • Identify required clinical trends and conditions, analyze timelines of treatment, identify areas for improvement and communicate findings to relevant Departments within LiveOnNY as needed.
  • Assists departmental leadership and executive staff with performance improvement efforts
  • All employees, at every level, are empowered and expected to take initiative and be committed to participate in continuous quality improvement process
  • Other duties as deemed necessary

Qualifications:

Quality Assurance Nurse Qualifications:


Required:


  • Registered Professional Nurse licensed in the State of New York required
  • Bachelor's degree in Nursing from an accredited college or university required
  • Twothree years Critical Care experience (ICU, ED, OR) in a hospital setting
  • Three years related experience and/or training required.
  • Must have knowledge of medical terminology, anatomy, and physiology.
  • Ability to research information, draw conclusions, and make recommendations.
  • Computer literate (e.g., Word Processing, Excel, PowerPoint).
  • Good interpersonal, oral, and writing skills.
  • Must possess excellent critical thinking and creative problemsolving skills as well as a high level of maturity, stamina and selfmotivation
  • Excellent communication, computer and organizational skills

Preferred:


  • Masters Degree
  • Critical Care experience (ICU, ED, OR) in Hospital setting
  • Experience working with EMRs with chart review experience
  • Previous QA/PI experience
  • Experience with transplant patients, critical neurological patients
  • Experience with QA process such as RCA, debriefs, failure mode analysis
  • ART certification
  • ICD 10 coding experience

Additional Requirements:

Primarily on-site work with occasional opportunity for remote work at discretion of management.


Monday to Friday, however weekends and evenings as part of an on-call rotating schedule based on Departmental needs may be required.
