Responsibilities

  • Ensuring a seamless, accurate and highquality admission experience for residents and their families, the Director of Case Management will identify and report changes in resident status and collaborate with property leadership and team members in addressing such changes.
  • Developing and maintaining accurate client records, logging resident interactions and/or impressions, and maintaining requisite resident case notes
  • Coordinating ISP process, incident reporting and the Falls Management Program, as well as coordinating external services, such as rehab, homecare, psychological services
  • Organizes and facilitates internal case conferences as needed, working with staff to design and implement personcentered, evidencebased interventions to resolve clinical/compliance issues, motivate consumers, and determine the disposition of cases.
  • Assists the Program Director and case management staff in providing quality clinical and crisis management services across the assisted living community, primarily through consultation and training.
  • Develop and maintain a good rapport with Interdisciplinary Team to ensure open and effective communication
  • Participate in a variety of interdisciplinary team meetings to communicate about relevant resident information and facility processes
  • Interview recipients, review records, and confer with other professionals to evaluate individuals' mental and physical condition and to determine their suitability for participation in a specific program.
  • Providing specialized inservice training to team members and conducting specialized support groups and/or social education sessions for residents and their families.

Qualifications:


  • Experience working in an assisted living, skilled nursing facility or longterm care environment
  • Successful background in nursing, case work, social work or health advocacy role
  • Sharp attention to detail, exceptional followthrough and time management skills
  • Extremely organized, ability to excel in a fastpaced, everchanging, multitasking environment
  • Exceptional interpersonal and relationshipbuilding skills a must, possessing the ability to clearly and calmly relate to and communicate concisely with a diverse group of people
  • Demonstrate confident leadership skills

Pay:
$80, $115,000.00 per year
