Summary:

The Rehabilitative Service Associate (RSA) will provide service delivery and documentation as outlined in the treatment plan developed by the Qualified Mental Health Professional (QMHP).

This position will also provide coaching and mentoring to designated staff in the behavioral health clinic and overall agency relative to their duties to assist supported individuals.


Responsibilities:


  • Support the achievement of desired goals for supported individuals based in individual choice as outlined in the person's treatment plan.
  • Skilled in the delivery of rehabilitative services to adults.
  • Provide coaching and mentoring to agency employees in regards to quality of life for supported clients.
  • Participate in the discovery process for success of the agency. Team, staff and clients supported.
  • Support database management systems including entry of plans into the system. Support the QMHP on training for any changes to client plans.
  • Ability to provide CPR/first aid as needed.
  • Maintain files in accordance with applicable regulations and agency standards.
  • Knowledge of and adherence to agency polices and procedures.
  • Employee may be asked to perform other duties to support the agency.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to walk and use hands. The employee is frequently required to stand for a minimum of an hour. The employee is occasionally required to stand; sit; bend; reach with hands and arms and stoop, kneel, or crouch. The employee must occasionally lift and/or move, push and/or pull up to 50 pounds. The employee must have the ability to walk a moderate distance, greater than 250 feet. Specific vision abilities required by this job include close vision, distance vision, peripheral vision and ability to adjust focus.


Qualifications:


  • Skill in the delivery of rehabilitative services for adults.
  • The ability to work constructively with individuals receiving services, other providers of services and the community.
  • Ability to work effectively in a team environment.
  • Must be organized and have a high attention to detail.
  • Minimum one year of experience supporting adults with disabilities.
  • Effective Communication Skills
  • Must be at least 21 years of age.
  • Highschool diploma or equivalent.
  • Valid driver's license and proof of insurance.
  • Mathematical knowledge to include basic addition, subtraction, division, multiplication, and computing averages.
  • DSP training/certification or completion within three months of employment.
  • Microsoft Office skills such as PowerPoint, Publisher, Excel, and Word
This position is a full-time, hourly, non-exempt position, mostly weekdays. Evening and/or weekend hours may be required.
