THE POSITION:


The Wayne County Area Agency on Aging has a rewarding position for a patient, compassionate and organized person who enjoys assisting senior citizens to find services and support to stay active and safe, while remaining in the community setting of their choice.

DESCRIPTION OF WORK:


An Aging Care Manager 2 works with older adults from initial contact with individuals, their families and professionals, through ongoing care management responsibilities.

Responsibilities include specialized duties (Protective Services, Care Transitions, Person-Center Counseling, etc.) You will have the opportunity to maintain a caseload of consumers and be involved in necessary areas of assistance.

You may regularly coordinate services with physicians, community agencies, financial institutions, etc.

Work includes managing multiple tasks and close attention to detail, which includes completion of paperwork, work assignments and meeting deadlines.


Work Schedule and Additional Information:


  • Fulltime employment.
  • Work hours are 8:00 am to 4:30 pm, Monday
  • Friday, with 60minute lunch.
  • Oncall hours when required.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY:


QUALIFICATIONS

Minimum Experience and Training Requirements:


  • Six months as an Aging Care Manager 1 or a County Caseworker 1;
    or:
  • Successful completion of the County Social Casework Intern program;
    or:
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field;
    or:
  • An equivalent combination of experience and training.

Other Requirements:

  • You must be able to perform essential job functions.

Legal Requirements:


  • You must pass a background investigation.
  • A conditional offer of employment will require a drug screening.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
