Description

The Intensive Prevention Services (IPS) Case Manager will assist at-risk youth in achieving socially and academically through intensive comprehensive programming; and prevent entry or re-entry into the juvenile justice system.


The IPS program is an after-school initiative funded by the City of Philadelphia Department of Human Services that provides a comprehensive, intensive early intervention services for youth exhibiting high or at-risk behaviors.


Responsibilities:


  • Conduct an initial assessment with the client/family.
  • Collect information about the client/family through use of interview, case history, and observational techniques.
  • Make mandatory weekly home visits, often with the assigned Counselor.
  • Link families to the parent education component of the program and work to involve families in planned family program activities.
  • Collaborate with team members to create Individual Service Plan for youth.
  • Provide program elements that meet the needs of youth and families in collaboration with IPS Counselors.
  • Lead monthly case review meetings with the team, ensuring that each child's progress towards meeting driven outcomes.
  • Consult with IPS Counselor to determine need for referrals, especially for substance abuse and mental health needs and make those referrals. Assures all appropriate referrals are made and followed up successfully.
  • Collaborate with IPS Counselor and team members to implement stepdown and discharge process.
  • Document all casework services provided for each family including, but not limited to, family assessments, youth's IPS, and monthly statistical and narrative reports.
  • Make court appearances with all youth on assigned caseloads.
  • Participate in all necessary interdisciplinary functions and meetings.
  • Maintain excellent relations with the assigned IPS Counselor and the team members., attend weekly supervision
  • Assist with transportation of clients as needed
  • Other duties as assigned

Qualifications and Experience Requirements:


  • Bachelor's Degree in relevant field (social work, behavioral health counseling, juvenile justice, education)
  • At least 2 years of experience working with youth or young adults.
  • Strong interpersonal & communication skills
  • Demonstrated ability to work as part of a team and in collaborative environment
  • Must have high proficiency with computer software, including Microsoft Word, Excel and Outlook.
  • Excellent organizational skills, ability to effectively handle documentation and paperwork


  • Bilingual

  • Spanish preferred, but not required
  • Must have excellent verbal and written communication skills.
  • Strong interpersonal skills and demonstrated ability to interact with individuals of different racial, cultural, educational, and economic backgrounds
  • Must have outstanding leadership and motivational skills.
  • Must be able to work as part of a team, be reliable and organized.
  • Possess or ability to secure driver's license and insurance (preferred)
  • Ability to secure health clearances every two years and TB once (required).

Pay:
$38,000.00 per year
