Overview:


The Team Leader will serve as the senior clinician for the Residential Pathways Programs overseeing the Pathways Safe Haven and Pathways Home programs on the fifth and sixth floors.

They will have overall responsibility for the direct supervision of employees on their team and will oversee the coordination and quality of services for clients in the programs.

They will develop a cohesive team by hiring and training dynamic clinicians, providing coaching, and mentoring to team members.

They will act in a player-coach role to provide direct clinical services to clients to assist with independent living skills, crisis management, medication management, and symptom management.

The Team Leader will routinely interface with other Team Leaders, Managers, the Executive Team and external partners to enhance communication, and cooperation/collaboration across programs/departments and community partners to improve the quality and comprehensiveness of services provided.

The Team Leader will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance use utilizing a harm reduction approach.


Responsibilities:


  • Complete weekly supervision with employees to provide feedback and followup on employee duties and responsibilities.
  • Model a clientcentered, recoveryoriented approach to services that includes the use of evidenced based practices, i.e. trauma informed care, motivational interviewing, and harm reduction techniques
  • Assess team's morale and continue to work towards keeping employee and team culture positive.
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated.
  • Support with tracking capacity for the programs to ensure that beds are filled.
  • Assess client's personal, medical, emotional, social, and environmental situation through meeting with clients in their units and in the office.
  • Assist clients in identifying signs and symptoms of decompensation and need for hospitalization. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced.
  • Support team members with crisis interventions such as petitioning and wellness checks.
  • Support team members with building engagement skills and assessment related to substance use and identifying stages of change with clients.
  • Coordinate with team nurse/provider/pharmacy to ensure client medication accuracy
  • Model effective communication within the team model for a multidisciplinary approach to client care.
  • Assist the team with organizing appointments for clients to ensure there is a plan for them to complete follow through with needs.
  • Run regularly scheduled team meetings and convey information/changes that are coming from management to the team.
  • Assist with hiring, onboarding new employees and ongoing training of staff to provide quality services to clients.
  • Monitor employees' direct service hours and ensures that direct reports and the team are meeting direct service targets.
  • Maintain documentation and ensure charts are in compliance with Medicaid, Commission on Accreditation of Rehabilitation Services (CARF), HUD, and organization standards.
  • Monitor Relias Trainings to ensure staff are compliant with annual trainings.
  • Work with the Financial Services Department to finalize client's rental payments, submit client budgets and BMFs which includes making adjustments as needed
  • Monitor team expenditures of client assistance and client loan funds.
  • Evaluate program's effectiveness in meeting deliverables. Recommend modifications in service structure or operations that affect the program. Develop plans to improve program performance.
  • Partner with CES, HMIS, and other Trilogy programs to review, admit, and discharge clients into the Residential programs.
  • Support with tracking documentation due dates (IMCANS, annual paperwork etc) and completion of needed documentation with clients.
  • Plan and facilitate special activities and outings for the residents.
  • Attend department meetings to stay compliant with team updates and trainings.
  • Perform other related duties and/or projects as assigned.

Qualifications:

  • Master's Degree in Social Work, Psychology, Counseling, or other closely related field or otherwise meet QMHP requirements
  • Minimum two years experience working with people with mental health and/or substance abuse diagnoses
  • Supervisory or leadership experience highly preferred
  • Experience working with people with mental health and/or substance abuse diagnoses in residential setting preferred
  • Experience and expert knowledge related to harm reduction services preferred
