Overview:


The Recovery Counselor - CTI will provide community support, care coordination, and referral services to clients in the Critical Time Intervention Project.

Services will include community-based visits for individuals based on their need and what phase of the project they are in.

The RC will support in facilitating case management and care coordination, referrals to more intensive Trilogy services, and other agencies in the community.

They will facilitate services that ensure successful and independent transition into stable housing by advocating for clients and linking them to external services as needed.

The RC will have strong engagement skills and be able to develop relationships with clients to support in getting connected with services to maintain stable housing.

They will manage a caseload of CTI clients and support in tracking of information needed for the project.

They will provide client-centered, strength-based, and trauma-informed case management to clients with severe mental illness and co-occurring substance use disorders, utilizing a harm reduction approach.

Treatment will be implemented via home, community, and office visits based on an assessment of needs, and with 60-75% of all community support services done in the community.


Responsibilities:


  • Work with a caseload of clients from the CTI project through the 3 phases of the project to provide housing stability and community linkage.
  • Assist clients with referrals and access to specialty care and community resources as needed.
  • Coordinate with the client's treatment team and support system to develop recovery and plans for maintaining stable housing.
  • Ensure continuity of care by assisting clients in connecting to external resources and services.
  • Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
  • Advocate on client's behalf and empower clients to advocate on behalf of themselves when appropriate.
  • Actively participate in team meetings and staffing related to CTI clients and provide effective communication within the team model for a multidisciplinary approach to client care.
  • Coordinate care with interdisciplinary staff, outside entities, and family members to ensure continuity of care for clients.
  • Complete documentation for progress notes, assessments, exits, and tracking for the CTI project and documentation within Carelogic including notes.
  • Assess client's personal, medical, emotional, social, and environmental situation through home, community, and office visits.
  • Assist clients in identifying signs and symptoms of decompensation. Work collaboratively with clients to identify coping skills to manage symptoms.
  • Assist clients in learning and improving independent living skills, i.e., personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items.
  • Perform other related duties and/or projects as assigned.

Qualifications:

  • Must have one of the following requirements:
  • Any individual who is 21 years of age that has a GED or high school diploma and demonstrates all of the following:
  • Skill in the delivery of rehabilitative services to adults
  • The ability to work within a provider agency's structure and accept supervision; and
  • The ability to work constructively with individuals receiving services, other providers of service, and the community; or an individual qualified as a Peer Support Worker
  • Valid Illinois Driver's License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
  • Experience working with people with mental health and/or substance abuse diagnoses preferred
  • Experience in communitybased services preferred
