Administrative Asst III:

  • The Treatment Team Coordinator performs complex (journeylevel) administrative support work and routinely interacts with treatment team members. The Treatment Team Coordinator is a member of the treatment team and plays a vital role during treatment planning meetings to gather input from all disciplines. The Coordinator will gather and compile this information into written drafts known as the Person-Centered Recover Plan (PCRP). Works under general supervision, with moderate latitude for the use of initiative and independent judgement.
  • Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

Essential Job Functions:


Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.


  • Collects, synthesizes, transcribes, and enters data into the medical record documenting all treatment recommendations agreed upon by treatment team members, patients, Local Mental Health Authorities, and relevant others (legally authorized representatives, family members, etc.) into a comprehensive individualized treatment/recovery plan for each person in service. Recovery plans demonstrate an understanding and appreciation of Person-Centered Recovery planning concepts and are written, as much as possible, in a way that reflects individual goals, strengths, preferences as well as person centered interventions
- that are tailored to specific individual needs/goals.

  • Completes data entry into medical record (comprehensive treatment/recovery plans and plan updates). Entry of information is accurate and timely and reflects treatment goals, interventions, and staff assignments. Treatment/recovery plans entered in the electronic medical record reflect knowledge, understanding, and competency in relation to regulatory standards (Centers for Medicare and Medicaid Services-CMS and The Joint Commission-TJC) regarding the essential
- elements of a multidisciplinary treatment plan.

  • Develops and maintains a tracking system to ensure that treatment/recovery plans for each individual in service are completed in a timely manner, are person specific, and are updated when changes are recommended by the treatment team.
  • Performs administrative duties to include coordinating, scheduling, and attending treatment team meetings. Maintaining files (electronic and paper). Participates in all mandatory training as well as any required program or specific training in order to meet requirements and ensure skills and knowledge are maintained.
  • Serves as a liaison/contact person for program staff, patients and centralized rehabilitation staff through direct communication and exchange of information in order to assure that patient's PCRP interventions are documented. Performs other duties as assigned. Knowledge Skills Abilities:

Knowledge of office practices and administrative procedures.

  • Knowledge of HIPPA and confidentiality rules.
  • Knowledge of the Electronic Medical Record (EMR).
  • Proficient knowledge and skills in computer use and the written language including spelling, punctuation, medical as well as psychiatric terminology.
  • Skills in the use of standard office equipment and software.
  • Ability to synthesize large amounts of information into a clearly written and
- concise document.

  • Ability to coordinate team members so that plans and reviews are finalized according to policy.
  • Ability to be productive and responsible in a professional environment.
  • Ability to communicate effectively using both oral and written methods of
- communication.

  • Ability to multitask and work under pressure.

Initial Selection Criteria:

Graduation from a standard senior high school or equivalent.
