Health and Nursing Care Professionals (m/f/d) or Pediatric Health Care Professionals (m/f/d)

... as Care Assessors (m/f/d) in full-time. After completing the basic training, we also offer various part-time models.

The main areas of deployment are the rural and urban districts of Pforzheim, Enz and Calw. A residence within the deployment area is a requirement.

Your tasks with us:

Individual case assessment based on the Social Code Book XI in a home environment, in facilities or based on file review, including recommendations for services, such as medical rehabilitation, home environment improvement measures, and aids.

What you bring with you:

Current professional experience in one of the mentioned nursing professions

In-depth knowledge in the field of disease theory

Advanced training, an academic degree, or leadership qualification and experience are advantageous

Structured work style and good self-organization
