In our Center for Internal Medicine, we are looking for a qualified case manager (m/f/d) for the nephrology department.

Your tasks:

Coordination and management of inpatient bed planning and treatment processes
Admission and discharge planning
Planning of post-hospital care, in collaboration with the social services department
Management/monitoring of length of stay in cooperation with the medical service
Interface between nursing staff, doctors, therapists, medical controlling, and social services
